I read this Rocket Lab "post-flight analysis" of the Electron launcher earlier today.

As I came to analysis section about the handling of flight telemetry (not turning ON the Forward Error Correction (FEC) before launch), I had an AMSAT-NA moment.
I remembered Phil Karn, KA9Q at the annual AMSAT-NA Conferences in 1990s ...
pushing that Forward Error Correction (FEC) to be included in future telemetry designs.
The general reception, 20+ years ago was mixed (Why change?), but AO-40 saw its usage.

So after 20 to 25 years ... Rocket Labs has shown that the "human component" continues to exhibit unacceptable failure rates.
