callbook servers needed for UK, DL, France, Japan, and others

Good day,

This is the second appeal to worldwide operators for cooperation is
assembling a unified list of call book servers available via the WWW.
If you have additions, deletions, or ideas for this list, please forward
them to me directly.

Thanks to all those with corrections and deletions from the original
post.

I see this as an evolving FAQ, and decided on these groups because of
their international audience. I will repost as a FAQ if I get enough
response to fill in the gaps.

If someone from Germany and the UK would send a note telling me why the
old sites seem to be dead or now commercial, I will include an
explanation in the FAQ.

Thanks for your help, and 73 from Mike in Fort Myers, FL
